1. Level of consciousness

2. Cyanosis (bluish skin/mucous membranes)

3. Stridor (high-pitched inspiratory sound)

4. Air entry (breath sounds on auscultation)

5. Chest wall retractions (subcostal, intercostal, suprasternal)
