๐Ÿงฌ Chapter 417: Peritoneal Malformations

Nelson Textbook of Pediatrics 22nd Edition โ€” Peritoneal malformations include congenital peritoneal bands (e.g., Ladd bands in malrotation), omental and mesenteric cysts (lymphatic malformations), internal hernias, peritoneal encapsulation, and omental duplication. Clinical features: abdominal pain, distention, palpable mass, intestinal obstruction (partial or complete). Diagnosis: ultrasound, CT scan. Treatment: surgical excision, lysis of bands.

๐Ÿ”‘ Key Principles โ€” Peritoneal Malformations (Nelson Ch.417)
โ€ข Congenital peritoneal bands: Can cause bowel obstruction (internal hernia, volvulus). Ladd bands in malrotation โ†’ duodenal obstruction.
โ€ข Omental & mesenteric cysts: Lymphatic malformations. Present as asymptomatic mass or with pain/distention/obstruction. Risk of torsion, hemorrhage, infection.
โ€ข Internal hernias: Bowel protrudes through mesenteric defect โ†’ closed-loop obstruction, strangulation.
โ€ข Peritoneal encapsulation: Accessory peritoneal sac covering small bowel. Usually asymptomatic, may cause obstruction.
โ€ข Treatment: Surgical excision of cysts, lysis of bands, reduction of hernia, repair of defect.
