Hallmark: encephalopathy (altered mental status, irritability, behavioral change) + polyfocal neurologic deficits. MRI: large, fluffy, poorly demarcated T2 lesions in white matter and deep gray matter (thalami, basal ganglia). MOG antibodies in 50-60%. Treatment: high-dose IV methylprednisolone, IVIG. Usually monophasic.
